For your convenience, please complete your Chiropractic Intake Paperwork, or your Chiropractic Infant Intake Paperwork (for children under 2), in the comfort of your own home.
You may bring it to your first appointment or fax it to 480-704-6617.
Your First Visit - will consist of completion of your intake paperwork if not completed prior to your appointment. The doctor will conduct: a brief consultation, a complete history, a thorough Chiropractic examination with appropriate orthopedic and neurological testing. A computerized scan and / or X-Rays may also be used to help assess your healthcare needs. Different comfort measures may be used depending on your Chiropractic needs.
Your Second Visit - the doctor will go over the results of your first visit. The doctor will give Chiropractic treatment recommendations based on your needs and healthcare goals. A care plan is agreed upon and Chiropractic care may begin at this point.
Subsequent Visits - you and the doctor will follow the agreed upon care plan until your Chiropractic healthcare needs or goals are achieved.
